Role OverviewThe Care Manager assists members in achieving their optimal level of health through self-management, engaging members and providers to assess and establish individual member goals.
What You Will Do
Assess members, complete comprehensive person-centered assessments, coordinate physical, behavioral health, and social services, provide medication management, and identify problems/barriers for care coordination.
Why It Might Be a Fit
The ideal candidate will have experience working with the behavioral health population, proficiency in Microsoft Tools and Medical Record systems, and strong organizational and time management skills.
Requirements
- Minimum of 3 to 5 years experience with the behavioral health population
- Experience working with individuals in Home Health
- Managed Care experience
- Ability to work with specific care teams and other engagement community partners
- Case management experience, preferably within a managed care organization
- Current, unrestricted RN Pennsylvania or Compact license in good standing
