Job Description
The Care Manager is an essential member of the CBES Home Care Interdisciplinary Team, working collaboratively with the CBES Team RN and other members of the Interdisciplinary Team to conduct assessments and develop comprehensive service plans to support elders' goals.

Requirements

  • Conduct initial and periodic face-to-face or telephonic assessments with elders
  • Develop and implement a comprehensive service plan
  • Coordinate service provision and collaborate with vendors and other agencies
  • Participate in hospital or Skilled Nursing Facilities discharge meetings
  • Conduct wellness check phone calls and/or emergency home visits
  • Identify elders in at-risk situations and report to crisis intervention agencies
  • Act as an advocate for elders and assist with housing issues, public assistance, insurance, and financial benefits
