Care Manager RN | Part Time | Mon-Friday 9a-1p Rotating Weekend and Holiday Schedule At Meritus Health, we believe that care doesn't end at discharge—it evolves. As a Care Manager RN, you'll be a key driver of clinical coordination and patient advocacy, guiding individuals through their acute care journey and beyond.

Requirements

  • Collaborate with the care management team and healthcare providers to coordinate patient care during acute episodes
  • Assess patients for care coordination needs and facilitate the physician's plan of care
  • Develop and implement discharge plans that support recovery and continuity of care
  • Apply evidence-based practices and utilization review standards to guide care progression
  • Coordinate with community resources and external providers to support post-discharge needs
  • Communicate effectively across disciplines to ensure optimal clinical, financial, and patient satisfaction outcomes
  • Navigate electronic medical records and contribute to documentation that supports regulatory and revenue cycle requirements
