Job Description
Adventist HealthCare is seeking an experienced Care Navigator Supervisor to lead a team in ensuring effective discharge plans and minimizing delays to care. The ideal candidate will have 3-5 years of experience in patient navigation, discharge planning, or care coordination, and a Bachelor's degree in nursing or social work. The role requires strong leadership skills, excellent communication, and a passion for delivering high-quality patient care.

Requirements

  • Bachelor of Science in nursing or social work required, Master’s degree preferred
  • 3 – 5 years’ experience in patient navigation, discharge planning, or care coordination
  • At least one year of prior leadership experience preferred
  • Maryland RN licensure
  • Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC
  • Active American Heart Association Basic Life Support (BLS) certification required
