Role OverviewThe Care Palliative Coordinator is responsible for facilitating a seamless transition for patients discharging from a facility setting to palliative care.
What You Will Do
Execute a weekly, monthly, and quarterly strategy to increase market share through key account development, assist with preparation for accepting care of the patient post discharge, and coordinate other services for the patient with ancillary service providers.
