Job Description

The Care Team Specialist serves as the behind-the-scenes point person who ensures consistent communication among a client’s care team, both within the organization and with external providers, to support optimal care.

Requirements

  • Bachelor’s degree in psychology, social services, sociology or other related fields desired or comparable work experience preferred
  • One year experience in health and human services, or related field
  • Ability to work in a fast-paced, collaborative and team-oriented environment
