What You Will Do
Assists patients in the process of navigating post-acute care with an overall goal of creating a positive impact on patient outcomes and referral source satisfaction. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care.

Must be a graduate of an approved school of nursing, therapy or social work. Must be licensed in the state where they currently practice. Must have two years' demonstrated field experience. Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
