Role OverviewThe Transitional Age Youth (TAY) Coordinator provides case management services in collaboration with the Transition Coordinator and other cross-functional teams to support young persons in transitioning into adulthood.
What You Will Do
The TAY Case Coordinator will focus on building a partnership with young persons to create a solid foundation for the transition into adulthood through intensive case management services; housing stabilization; job coaching and access to supported employment opportunities; resource procurement and advocacy.
