We are looking for a Case Management Assistant to assist the Case Manager in identifying member needs, initiating member contact, and providing education and coordination of community resources to promote optimal health.

Requirements

High school diploma or GED
2 years of case management, community outreach, healthcare, and/or health insurance experience
3 years of general administrative experience
2 years of Call/Contact Center experience (preferred)
