Job Description
Endeavor Health Services seeks a Behavioral Health Team Case Manager to provide direct support to individuals with mental health needs, connecting them with community-based services and advocating for their needs.

Requirements

  • Bachelor’s degree in Behavioral Science or a related field
  • Minimum of 1 year of experience providing direct services
  • At least 2 years of crisis intervention experience
  • Strong knowledge of behavioral health needs and community resources
  • Ability to work independently and as part of a multidisciplinary team
  • Excellent communication, advocacy, and organizational skills
  • Valid New York State driver’s license and reliable transportation
