Job Description
This position requires working in the community and performing home visits. The Case Manager will coordinate services to maximize consumer recovery and independence, and provide clinical and program consultation to direct care staff.

Requirements

  • Bachelor’s degree in a human services field from an accredited institution required
  • LLBSW, LBSW, LLMSW, LMSW, LMFT preferred
  • Prior case management experience in a mental health or rehabilitation setting preferred
  • Valid Michigan Driver’s License
