Job Description
The Case Manager will provide ongoing recovery-focused community rehabilitation services to individuals with severe and persistent mental illnesses at the Frese Step-Down Center. The role involves coordinating delivery of necessary evaluations, developing Interdisciplinary Service Plans, and assisting individuals to develop skills and minimize the effects of psychiatric symptoms.

Requirements

  • Associates Degree in a Human Service field
  • Prior experience in case management working with individuals with severe and persistent mental illnesses preferred
  • A car and a valid Ohio driver's license and insurability through Agency’s insurance carrier
  • Ability to work a flexible work schedule including evenings, weekends, and holidays
