The Case Manager will be responsible for daily management and coordination of care for patients in the hospital with identified skilled post-acute care needs.

Requirements

  • RN 2 years of clinical experience
  • 2 years of inpatient acute hospital case management experience or experience in either acute or non-acute case management
  • Current California RN License
  • Current BLS for Health Care Provider from American Heart Association
