Job Description
HealthRIGHT 360 is seeking a Case Manager for the Prototypes Pre-Filing Diversion Program (PFDP) to work with clients to connect them to essential community-based services and prevent incarceration.

Requirements

  • Maintain a caseload of up to 30 clients
  • Collaboratively develop and implement an individualized care plan
  • Assess clients and provide linkages and referrals to needed services
  • Accompany clients to medical and mental health appointments
  • Research life skills and community participation for clients
  • Monitor legal issues and make referrals to overcome barriers to housing and services
  • Coach and advocate for clients to access mental health and addiction treatment
  • Help clients gain, restore, and improve daily independent living skills
