Role OverviewThe Case Manager will work with residents in supportive housing to provide case management services, including referrals for specialty care, coordinating appointments, and completing assessments and service plans. The role requires direct face-to-face contact with consumers and the general public on an ongoing basis.
What You Will Do
Responsibilities include navigating client services, completing assessments and service plans, meeting with clients, ensuring clinical and administrative documents are completed accurately, providing crisis intervention services, and working with residents to elevate their daily living skills.
Why It Might Be a Fit
This role requires a BSW or BA in human services, with a minimum of two years of experience with homeless youth living in supported housing. CASAC or CASAC-T is preferred. Good written and verbal communication skills, basic computer and data entry skills, and the ability to undergo pre-employment screenings are also required.
