Job Description
The Case Manager provides professional and individualized participant case management services to assist participants in addressing the underlying causes of their homeless condition and removing barriers to independence.

Requirements

  • Meet with participants weekly to assess, support, and motivate them to work towards their goals
  • Develop individual service plans and monthly action plans with participants
  • Strengthen job readiness skills through coaching
  • Assist clients in developing a personal budget and savings plan
  • Maintain accurate client data in HMIS/Clarity and paper file
