Job Description
Manage client care for individuals requiring multiple assistance sources, providing case management services, assessment, planning, advocacy, and support. Establish and maintain cooperative relationships with community resources and service providers. Advocate on behalf of persons served to receive required services and become successful in the community.
Requirements
Bachelor's degree in behavioral sciences
Two years of experience in counseling
Knowledge of principles and techniques of social service case management
Knowledge of community resources
Considerable knowledge of federal and state laws and regulations regarding client confidentiality
Excellent communication skills
Ability to establish and maintain cooperative professional relationships
