Role OverviewThe Case Manager is part of the clinical team providing ongoing care coordination and aftercare planning to individuals who are receiving residential services.
What You Will Do
Meet with clients weekly to provide care coordination and aftercare planning, conduct assessments to identify clients' needs, strengths and barriers to attend treatment plan goals, and facilitate referrals and linkages to all service needs that clients have.

Requirements
- Bachelor's Degree in human services, psychology, sociology, health services, or related field
- At least one year working in a substance use program
- Ability to communicate and represent Dimock in a professional manner in the community and within the organization
- Intermediate computer skills (Work, Excel, PowerPoint)
- Ability to be flexible and juggle multiple priorities
