Role OverviewThe Case Manager role is responsible for providing service provision and housing assistance for participants engaged in programs housed out of the Bannock Youth and Family Center. The role involves conducting needs-based assessments, community outreach, providing case management services, and assisting with housing navigation.
What You Will Do
The Case Manager will conduct needs-based assessments, provide case management services, assist with housing navigation, and support housing retention. The role also involves connecting participants to income resources and/or public benefits, determining rental assistance, and attending meetings/trainings within the community.

Requirements
- Bachelor's degree in human services, social work, or a closely related field
- At least one year of direct, full-time case management experience working with youth who are experiencing homelessness
- Experience working with literally homeless populations, youth, and youth-headed households
- Crisis intervention skills
- Colorado driver's license and state-mandated automobile insurance
