Job Description
The Case Manager is responsible for monitoring clients' weekly activities, assisting with employment and housing, and coordinating clinical case management services. They will also ensure timely information is shared with referral providers and family members, and complete all required paperwork.

Requirements

  • Develops and monitors Individual Program Plan for clients on caseload
  • Meets regularly with each client to discuss progress with program requirements and personal goals
  • Maintains ongoing communication with referral providers and family members
  • Completes all required paperwork for each client on caseload
  • Attends necessary meetings, conferences, and training
  • Ensures clients are scheduled within 2 business days of receiving the referral
