Job Description
Role Overview

The Case Manager-CSJ provides case management and other direct services to clients throughout Santa Clara County. The role reports to the Program Manager and is part of HomeFirst's Housing Stabilization Department, which aims to end and prevent homelessness for at-risk households. The Case Manager-CSJ will work with community partners, coordinate with team members, and provide individualized case management action plans to help clients achieve their goals.

What You Will Do

Provide case management and direct services to clients, coordinate with team members, collaborate with community partners, and participate in property management meetings. The Case Manager-CSJ will also maintain thorough records of case management activities, document progress notes, and complete incident reports as needed.

This role requires strong written and oral communication skills, ability to manage multiple projects, and a sense of accountability. The Case Manager-CSJ should be able to develop and maintain priorities, meet deadlines, and work as a member of a team. Bilingual Spanish or Vietnamese speaking or bicultural is preferred, but not required.

Requirements

  • 2 years of paid or volunteer experience in social service or related human services
  • Bachelor of Arts in Social Work (BASW) or related human services degree preferred
  • Valid CA driver license and ability to be added onto the agency fleet insurance
  • Reliable personal vehicle and auto insurance
  • Ability to lift up to 25 lbs. on occasion
  • Ability to use keyboard and read computer screens for extended periods
  • Ability to stand/sit for extended periods, kneel, reach and bend related to the completion of duties
