The Case Manager provides comprehensive support and coordination of care throughout the addiction recovery process, assessing client needs, connecting clients to resources, and monitoring progress toward recovery goals.

Requirements

  • Assess treatment needs and develop, monitor, and evaluate treatment plans and progress
  • Coordinate wellness checks, doctor appointments, and other necessary services
  • Maintain current contact lists of community resources and coordinate referrals
  • Communicate effectively in writing and interpreting client legal commitments
