Role OverviewThe Nutrition Case Manager provides screening and assessment services for home delivered meal applicants within Sarasota County and supports congregate meal dining site operations and additional nutrition programs. This position meets with seniors at their residential location to assess what programs and assistance are available to the senior.
What You Will Do
Respond to referrals for home delivered meals, administer the DOEA 701A/701C assessment form electronically, and complete all case file documentation. Develop and promote relationships with service providers and businesses to assure understanding of the organizations mission.

Requirements
- Case Management Experience
- Management experience is strongly preferred
- Experience presenting social, recreational, and educational programs to adults is desirable
- Intermediate computer skills are required
- Must have experience using Microsoft Office Suite (Word, Excel, Outlook) and other basic computer software and hardware
- Bilingual preferred (English/Spanish)
