Job Description
The Case Manager I renders professional intervention to clients in need of assistance through comprehensive support services. The position involves providing case management services, assessment, and intake of participants, developing individualized case plans, and facilitating connections to community resources and services.

Requirements

  • Bachelor's degree in social work, Human Services, Counseling, or a related field or AA degree in social work, Human Services, Counseling, or a related field with two years' related field experience in City systems such as justice, public assistance, child welfare, housing or other relevant social service setting providing direct services
  • Proven experience in case management, counseling, or a related field, working with the target population or vulnerable populations
  • Ability to work flexible hours, including weekends, evenings according to the needs of program operation
  • Ability to conduct home/field visits and travel throughout the five boroughs as needed
  • Ability to utilize public transportation, climb stairs, walk
