Job Description
Case Manager II responsible for developing psycho-social assessments and service plans for children and families to ensure child safety and reduce child risk. Performs case management duties, advocates for entitlements, and provides one-to-one mentoring and coaching to promote economic mobility and family alignment.

Requirements

  • Bachelor's Degree
  • Experience in case management preferred
  • Experience engaging with child- and family-serving systems in the greater New York area preferred
  • Knowledge of Motivational Interviewing and coaching practices
  • Ability to work with adults towards economic mobility goals
  • Knowledge of best practices in the field and connections with systems addressing economic mobility
  • Excellent organization skills
  • Ability to manage a caseload of up to 12-13 families effectively and efficiently
  • Experience working with individuals who have experienced trauma and crises
  • Ability to work flexible hours (evenings/Saturdays) as assigned
  • Ability to conduct home/field visits and travel throughout the five boroughs as needed
  • Ability to lift up to 10lbs
