Job Description
The Case Manager II will have a focus on family at our shelter. Case Managing Families to help them progress in life and eventually move out of the shelter into other supportive services or into self-sufficiency.

Requirements

  • Assess what other resources and agencies would benefit the resident and make referrals accordingly.
  • Maintain accurate written case notes on each resident interviewed.
  • Meet with Families assigned to you weekly for updates and guidance.
