Job Description
The Case Manager develops and implements care plans for clients that coordinate home care services and follow physicians' orders.

Requirements

  • Performs client intake assessments
  • Develops a plan of care
  • Routinely reviews pertinent client medical data
  • Investigates and takes appropriate actions on client or employee concerns/complaints
  • Ensures that aides are properly oriented and trained
  • Performs periodic supervisory visits for aides
  • Instructs clients, family members and primary care givers
  • Ensures effective coordination of client home care services
  • Provides input for formulation or modification of agency policies
  • Participates on the advisory board or other agency quality assurance committees
