Job Description
The Case Manager develops and implements care plans for clients that coordinate home care services and follow physicians' orders.
Requirements
Performs client intake assessments
Develops a plan of care
Routinely reviews pertinent client medical data
Investigates and takes appropriate actions on client or employee concerns/complaints
Ensures that aides are properly oriented and trained
Performs periodic supervisory visits for aides
Instructs clients, family members and primary care givers
Ensures effective coordination of client home care services
Provides input for formulation or modification of agency policies
Participates on the advisory board or other agency quality assurance committees
