Job Description
Mentor Community Services is a mission-driven team providing community-based services for individuals with intellectual and developmental disabilities. The Case Manager role involves providing case management services, including intake, assessment, crisis intervention, and coordination with service providers.
Requirements
Bachelor’s degree in human services or equivalent in education and experience
One year of related work experience and knowledge of case management
Valid driver’s license, registration, and insurance
Current CPR/First Aid Certification
Ability to establish working relationships with individuals served
Demonstrated competence in verbal and written communication skills
Ability to handle crisis situations
