Job Description
Valley Hope of Norton has an exciting opportunity for a Case Manager to join our dedicated and passionate team! The Case Manager will work with patients and their families to provide continuing care, facilitate rehabilitation group sessions, and identify needed recovery resources.

Requirements

  • One plus years of experience in a similar position and/or healthcare environment.
  • Cardiopulmonary Resuscitation (CPR) certification.
  • Bachelor’s or graduate degree in behavioral sciences, education, or social work.
  • One plus years of work experience in substance abuse counseling industry.
  • Certified as Case Manager in the state of employment.
