We are seeking a qualified candidate for a Case Manager position to work with clients, assess their strengths and needs, plan a course of action, and provide services to achieve successful outcomes. The position is located at the East 119th Street Veterans Residence in New York, NY.

Requirements

  • Associate's degree or 60 college credits courses in Human Services field, or High School Diploma or GED with 2 years of relevant shelter experience.
  • Demonstrated ability in verbal and written communication, including basic computer skills.
  • Ability to manage client intake and assessment process, develop individual service plans, and document case management activities.
