Job Description
The Case Manager will interview and educate patients infected with communicable diseases, conduct surveillance activities, and refer patients to diagnostic facilities. The role requires establishing and maintaining communication with coworkers and community partners, managing patient data, and conducting quality assurance activities. The Case Manager will work with the Bureau of Tuberculosis Control to prevent the spread of tuberculosis and eliminate it as a public health problem in New York City.

Requirements

  • Bachelor's degree in health education, health, social, or biological sciences
  • Experience in health promotion or disease intervention/prevention program
  • Ability to work with patients and community partners
  • Effective communication and interpersonal skills
  • Ability to conduct surveillance activities and manage patient data
