Job Description
Queens Community House is seeking a qualified Case Manager with a strong commitment to social services and a passion for working with older adults. The Case Manager will build trusting relationships with clients and caregivers while engaging them in a collaborative, problem-solving process to address their unique needs and support their well-being.

Requirements

  • Bachelor of Social Work (BSW) degree required.
  • Minimum of 2 years of experience working with the gerontological population.
  • Bilingual in Russian required.
  • Demonstrated cultural competency and sensitivity to the diverse needs of older adults in the program's catchment areas.
  • Knowledge of communication needs related to visual and hearing impairments.
  • Strong multitasking and organizational abilities with a high level of energy and initiative.
  • Proficiency with various computer applications and software; knowledge of the STARS database preferred.
  • Ability to manage sensitive and confidential information with professionalism and discretion.
  • Strong analytical and critical-thinking skills with the ability to resolve complex client-related issues.
  • Excellent interpersonal, communication, and customer-service skills.
  • Demonstrated ability to build trust and maintain positive professional relationships.
  • Proven ability to follow through on assignments with strong attention to detail.
