Job Description
Queens Community House is seeking a qualified Case Manager with a strong commitment to social services and a passion for working with older adults. The Case Manager will build trusting relationships with clients and caregivers while engaging them in a collaborative, problem-solving process to address their unique needs and support their well-being.
Requirements
Bachelor of Social Work (BSW) degree required.
Minimum of 2 years of experience working with the gerontological population.
Bilingual in Russian required.
Demonstrated cultural competency and sensitivity to the diverse needs of older adults in the program's catchment areas.
Knowledge of communication needs related to visual and hearing impairments.
Strong multitasking and organizational abilities with a high level of energy and initiative.
Proficiency with various computer applications and software; knowledge of the STARS database preferred.
Ability to manage sensitive and confidential information with professionalism and discretion.
Strong analytical and critical-thinking skills with the ability to resolve complex client-related issues.
Excellent interpersonal, communication, and customer-service skills.
Demonstrated ability to build trust and maintain positive professional relationships.
Proven ability to follow through on assignments with strong attention to detail.
