Job Description
Community Care is hiring a Case Manager for a full-time opportunity with a hybrid weekday schedule and a $8,000 SIGN-ON BONUS. The position involves managing an assigned caseload of program members and providing comprehensive member assessments, care plans, and quality assurance monitoring.

Requirements

  • Bachelor’s Degree in a Human Services or closely related area required
  • One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred
  • Eligibility for long-term care functional screener certification required
  • Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage
