Job Description
The Case Manager provides support services and resource coordination for youth, adults, elders, and families in need of social services assistance, connecting them with housing resources, rehabilitation programs, healthcare services, and community support programs.

Requirements

  • Conduct initial intake interviews to determine program eligibility and service needs.
  • Complete comprehensive assessments and intake documentation to identify appropriate housing options, supportive services, and case management needs.
  • Develop individualized case plans with measurable goals and objectives.
  • Maintain accurate and timely case notes, documentation, hard files, and electronic records.
  • Ensure confidentiality and compliance with applicable laws, policies, and program requirements.
