Job Description
The Center for Human Development is seeking an experienced and creative Case Manager at its Two Rivers Recovery Center in Greenfield to develop and implement person-centered treatment plans, facilitate therapeutic groups, and provide community support coordination.

Requirements

  • High school diploma or equivalent required
  • Bachelor’s level required
  • Knowledge of substance use disorders and treatment
  • Experience facilitating groups
  • Must pass background screening process including CORI
  • Valid driver’s license and have a clear driving record check
  • Have reliable transportation, for occasional business use
