Job Description
Mentor Community Services provides community-based services for individuals with intellectual and developmental disabilities. Case Managers provide case management services, including intake, assessment, crisis intervention, advocacy, referral, and monitoring of families. They gather social history to assess family strengths and weaknesses, and existing problems, and coordinate care with outreach workers, health educators, nutritionists, and other service providers.

Requirements

  • Bachelor’s degree in human services or equivalent in education and experience
  • One year of related work experience and knowledge of case management
  • Valid driver’s license, registration, and insurance
  • Current CPR/First Aid Certification as required by state/program
  • Ability to establish working relationships with individuals served
  • Demonstrated competence in verbal and written communication skills
  • Ability to handle crisis situations
