Job Description
As a Case Manager for Homebase, you will provide case management services, crisis intervention, and advocacy for individuals and families facing a housing crisis. Responsibilities include completing intake interviews, establishing service plans, and advocating for clients to ensure receipt of entitlements and housing stabilization.
Requirements
Bachelor's degree in social work or a related field OR equivalent experience and skills
Knowledgeable of relevant state and county regulations
Minimum of two years' experience in case management, assessment, counseling, and crisis intervention
Strong verbal, written, and negotiation skills
Computer literate, specifically with Microsoft applications
