Job Description
As a Case Manager for Homebase, you will provide case management services, crisis intervention, and advocacy for individuals and families facing a housing crisis. Responsibilities include completing intake interviews, establishing service plans, and advocating for clients to ensure receipt of entitlements and housing stabilization.

Requirements

  • Bachelor's degree in social work or a related field OR equivalent experience and skills
  • Knowledgeable of relevant state and county regulations
  • Minimum of two years' experience in case management, assessment, counseling, and crisis intervention
  • Strong verbal, written, and negotiation skills
  • Computer literate, specifically with Microsoft applications
