Role OverviewThe Case Manager works with clients to assess their strengths and needs, plan a course of action, link them to services, counsel them, and monitor progress. They must assure that clients receive appropriate, competent, and professional information and advice, referrals, and advocacy services to achieve successful outcomes.
What You Will Do
Manage client intake and assessment, develop individual service plans, meet with clients to assist them in obtaining needed services, provide supportive counseling, and document case management activities.

Requirements
- Associate's degree or 60 college credits courses in Human Services field
- High School Diploma or GED with 2 years of relevant shelter experience
