Role OverviewThe Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in various settings.

Implement and transfer skills identified in the recovery treatment and support plan, utilizing a person-centered approach.

Requirements
- Bachelor’s degree in Social Work, Psychology, Criminal Justice, or similar human services
- Experience in the behavioral health field working with individuals with mental health/developmental disabilities
- Valid PA driver’s license with a clean driving record and reliable transportation
- Possesses or is able to obtain all necessary clearances and background checks to work with designated consumers
