Role OverviewThe Case Manager works directly with clients to conduct interviews, evaluate their needs, and collaboratively develop Independent Living Plans. This includes identifying and leveraging community resources, coordinating referrals to services, and assisting clients in relocating to permanent housing.
What You Will Do
Build professional relationships with clients, review client eligibility documents, create and manage client files, conduct intake interviews, develop and update Independent Living Plans, refer clients to services, support clients in overcoming barriers, track client progress, and provide data for required reports.

Requirements
- High School Diploma or equivalent
