Role OverviewThe Case Manager is responsible for developing, implementing, and monitoring Life Plans for individuals with developmental disabilities in a residential setting. This role ensures that services align with regulatory requirements, agency standards, and personal goals of each individual.
What You Will Do
Provides social services, develops and monitors Individualized Service Plans, participates in interdisciplinary team meetings, and collaborates with residential and clinical staff to ensure coordinated support.

Requirements
