Role OverviewThe Case Manager provides high-quality program-based case management services to families assigned to a homeless shelter. They assist clients in developing goals that lead to housing stability, monitor progress, and provide structure and service linkage to aid in transitioning from shelter to permanent housing.
What You Will Do
Develops Service Plans, ensures successful completion of assigned goals, provides case management sessions, monitors and provides community linkages, collects statistics, and completes assessments.

The ideal candidate will have a Bachelor's Degree with 2 years' experience working with homeless individuals or a combination of an Associate's Degree with 4 years' experience.
