Role Overview

The Case Manager is responsible for developing, implementing, and monitoring Life Plans for individuals with developmental disabilities in a residential setting. This role ensures that services align with regulatory requirements, agency standards, and personal goals of each individual.

What You Will Do

The Case Manager provides advocacy, coordination of services, and family engagement, while promoting dignity, independence, and quality of life for the people we support. The duties and responsibilities include providing social services, developing and monitoring Individualized Service Plans, and collaborating with residential staff and service providers.

Requirements

  • Bachelor’s Degree in Human Services or related field

  • Excellent organizational and time management skills
  • Proficiency in Microsoft Office and electronic record systems
  • Ability to maintain confidentiality and exercise discretion in handling sensitive information
