Role OverviewThe Case Manager will work in the Re-Entry Bureau, providing services to formerly incarcerated individuals and criminal justice-impacted individuals. The role involves conducting assessments, assisting with case management, creating individualized service plans, and providing counseling sessions. The Case Manager will also participate in meetings, prepare progress reports, and attend resource fairs.

Requirements
