Role OverviewThe Case Manager is a key provider of services within Volunteers of America's Case Management Model, working with clients to assess strengths and needs, plan a course of action, link to services, counsel, and monitor progress. This role is based at Webster House, a single room occupancy residence providing permanent supportive housing to formerly homeless single adults, many living with mental illness or HIV/AIDS, in Bronx, NY.
What You Will Do
Manage the client intake and assessment process to orient clients to the program and produce a comprehensive assessment of service needs. Develop mutually agreed-upon individual service plans, meet regularly with clients to assist in obtaining needed services and benefits, provide supportive counseling, and document case management activities in accordance with agency and funder guidelines.
