Role OverviewAs part of the interdisciplinary GCAOP treatment team, the Case Manager is responsible for guiding clients in the establishment of benefits and assistance in accessing outside resources.
What You Will Do
Provide case management linkage services, assist clients in obtaining California I.D. and immigration documentation, and provide rehabilitation services to improve daily living skills.

The Case Manager will work collaboratively with the multi-disciplinary treatment team, communicate with clients and families from a strength-based stance, and maintain active and effective liaisons with relevant community agencies.
Requirements
- Prior experience working with children and families from diverse ethnic, cultural, and socioeconomic backgrounds
- Prior experience working with seriously emotionally disturbed youth and their families to provide Targeted Case Management
- Experience participating in multidisciplinary teams
- History of strong writing ability in previous employment
