Role OverviewNexus Family Healing is seeking a Case Manager to join their team in Manteno, IL. The Case Manager will ensure the unique treatment needs and requirements of each assigned youth are met. This role is crucial to the success of the organization and its youth.
What You Will Do
The Case Manager will provide therapeutic services, including facilitating case conferences, developing and implementing individual treatment plans, and collaborating with family members and other stakeholders. They will also engage in counseling sessions and therapeutic interventions.

Requirements
- Bachelor’s degree in a clinical discipline (Psychology, Social Work, etc.)
- Strong critical thinking, organization, and communication skills
- Attention to detail and ability to exercise discretion with highly sensitive and confidential information
- MHP eligibility
- Minimum 21 years of age
- Valid driver’s license
- Pass IL DCFS criminal background check
- Participate in and complete a three-part Therapeutic Crisis Intervention exam and training
