Job Description
NAFI Connecticut is a dynamic multi-service agency that provides programs, consultation, and support services to adults, children, and families across Connecticut, Rhode Island, and New York. The Orange County Family Wraparound Program is a short-term comprehensive program that focuses on community supports, early interventions, home-based responses, and post-placement re-integration for children.

Requirements

  • Provide support and guidance to program families and youth around service plan goals
  • Provide linkages to community services to ensure that families receive benefits, entitlements, and needed services
  • Provide case management counseling to youth in the program
  • Assist with group activities, childcare, and respite, tutoring, and mentoring when needed
  • Provide transportation support for families to attend medical appointments and/or court
