Role OverviewThe Case Manager Mental Health Professional is responsible for developing and implementing individual treatment plans to maximize clients' independent community living skills. The role involves working with a team of behavioral health service providers to ensure a positive client-centered approach. The Case Manager MHP must complete documentation of mental health services provided in accordance with state and agency standards as applicable.
What You Will Do
The Case Manager MHP will provide independent program coverage, meet with assigned clients on a regular basis, and turn in daily activity logs. The role also involves assuring the coordination of services in the community to meet client needs, assisting clients with activities of daily living, and facilitating communications and coordination of services with other AID staff and professionals in the community involved in client care.

Requirements
- Bachelor's degree in human services or related field
- Some experience working in social services
- Bilingual experience
- Valid driver's license
- Minimum amount of liability insurance as defined by AID's Personnel Policy
