Job Description
Case Manager-Outpatient position provides direct service to individuals served, family members, and designated others, utilizing a Person-Centered process and Family Centered Practice Concepts. Coordinates, consults, and provides liaison work with physical health care providers and other agencies.
Requirements
Bachelor's Degree in Psychology, Social Work, Special Education, Counseling, or other mental health-related field
Michigan Department of Health and Human Services (MDHHS) Prepaid Inpatient Health Plan (PIHP)/Community Mental Health Services Programs (CMHSP) Qualifications as a Qualified Mental Health Professional (QMHP)
Possession of required state licensure and ability to meet privileging and credentialing guidelines
Valid Michigan driver's license and operable, insured automobile for authorized travel
